It’s about marriage equality, which to me, shouldn’t be such a big controversy, because it is as simple as we live in a country where we are free and equal. This song doesn’t only represent something real, but it represents a movement. And that’s power. When a song, especially a rap song has the ability to resonate and reach people on a less than superficial level, that’s beautiful. Macklemore, I think is one of those few artists who are not concerned about making hits, but making something the people can connect with. And that is damn hard to find in today’s music. His album, The Heist is a compilation of different tones, instruments, and sounds, and it’s frankly something that should be heard. Same Love to me, is one of those things that can’t always be outright said. Whereas on the one hand medical costs of treatment are rising, on the other malpractices and non compliance on the part of medical professionals and institutions compounds the problem and seriously questions the quality of health care being provided to citizens. However, before proceeding further it is important to understand what is exactly meant by the substandard quality of care. The substandard quality of care implies that one or more of the requirements mandatory under the federal regulations 42 CFR 483 . 13 involving resident behavior and facility practices, 42CFR . 15 involving quality of life or 42CFR 483 . 25 regarding quality of care are not complied with leading to actual jeopardy to the resident health or safety or having potential for causing more than minimal harm (HealthCare information, 2007). Any nursing home found with delivering substandard quality of healthcare or carrying significant deficiencies in its healthcare treatment plans would be required to immediately address the issue. The following personal experience presents an example of injuries from utilization control healthcare delivered resulting in grievous and permanent injuries to the patient. The problem here is about Lois Battles, my grandmother, who was diagnosed with Leriches syndrome, which is a vascular occlusion. Upon diagnosis, her doctor recommended surgery where a part of an artery was to be replaced with a Teflon graft. The program providing medical care to Lois was Californias medical assistance program, Medi-Cal that was responsible for controlling costs and authorizing treatment. As such, the approval and authorization from state was a requirement to perform surgery. When Lois’s doctors sought approval from Medi-Cal for the surgery, Medi-Cal agreed and also allowed ten days of post-surgery treatment in hospital. However, Mrs. Battles recovery did not take place as estimated and a day before due discharge date, the surgeon and his assistant felt that Mrs. Battles required additional eight days of recuperation in hospital. Medi-Cal was again approached for the approval for this extended stay, but on this occasion Medi-Cal differed from surgeon’s recommendations for eight days extension. The Medi-Cal representative, who was himself a certified surgeon, merely consulted a Medi-Cal nurse on the phone and after evaluating progress of Mrs. Battles on basis of her temperature, diet and bowel function, declined the request and approved only four day extension. In the process he neither reviewed any documents related to the case nor consulted any other vascular-surgery specialist. On denial by Med-Cal, the surgeon attending the case of Mrs. Battles decided not to push for the additional days of care and complied with Med-Cal directives by discharging her at the end of the stipulated period. Mrs. Battles than got an infection and was sent right back in to the hospital for treatment. If Mrs. Battles would have stayed the four extra days she may have been fine. So in order to be a successful organizational and business performance is the ability to manage the accessibility and processing of information. Accurate, timely and relevant data form the basis for planning, monitoring and decision-making processes across an organization. Healthcare, more than other industries, feels the pinch of dealing with substandard and inaccurate information because it is more data rich, data dependant and data sensitive than any other sector of the economy. The practice of medicine requires continuous availability across the various functions to ensure best care outcomes as well as a perpetual state of readiness for possible changes in patient demands, competitive scenario, regulations or disease patterns and new outbreaks. Healthcare information technology forms a pivotal part of todays healthcare system. Improved quality of healthcare, patient safety, drastic reduction in medical errors and enhancement of care delivery is possible because of the induction of these solutions in the healthcare delivery environment. Today, healthcare IT solutions are enabling processes like planning, decision-making related to organizational development, strategic planning to drive growth, recruitment of competent personnel, managing employee benefits, payroll, intra-department communication, etc. One policy that has changed the health care delivery to shift cost responsibility toward the hospital was the Tax Equity and Fiscal Responsibility Act of 1982. This policy has changed hospital reimbursement under Medicare with the introduction of diagnosis-related groups (DRGs) (Sultz amp; Young, 2009). The DRG prospective payment system rewarded hospitals financially for efficient care (specifically shorter hospital stays) and reduced the incentive to consume (Sultz amp; Young, 2009). Another policy that placed the burden of responsibility on the hospital was the Consolidated Omnibus Budget Reconciliation Act of 1985 which required hospitals to provide care to everyone who presented in the emergency department regardless of ability to pay (Sultz amp; Young, 2009). This law was designed with good intent to reduce the â€Å"patient dumping† that had arisen from the DRG system, but it nevertheless increased hospital responsibility without additional financial support. More recently, the Deficit Reduction Act of 2005 required the Secretary of the Department of Health and Human Services to select at least two hospital-acquired conditions for which hospitals will no longer be reimbursed by Medicare (United States Congress, 2006). The purpose of this new law is to contain costs by penalizing hospitals for preventable conditions or complications resulting from substandard care. The designated conditions were to meet the requirements of being high cost and/or high volume, and reasonably preventable through the application of evidence-based guidelines (S. Rep. No. 1932, 2006). Eight hospital-acquired conditions have been selected thus far and at least nine additional conditions are up for consideration of being added to the list for 2009 (Center for Medicare amp; Medicaid Services (CMS), 2008). The way this law works is that if a patient develops a hospital-acquired condition or complication during their hospital stay that was not documented upon admission, Medicare will no longer reimburse the hospital for the treatment of the condition and the hospital will be forced to absorb the cost. Conditions included in this new rule particularly relevant to nursing include stage III and IV pressure ulcers, injuries from falls, and catheter-associated urinary tract infections. In light of these policy changes, hospitals are under intense pressure to improve quality of care and reduce costs. Many areas of vulnerability for hospitals such as patient safety, prevention of hospital-acquired conditions, and continuity of care, are directly related to nursing. However, instead of areas of vulnerability, it is better to view the current challenges of health care as opportunities for improvement.

There, at the boarding school, he meets a couple of boys who become friends: James Steerforth and Tommy Traddles. David doesnt complete his education because his mother dies and hes sent to a factory. There, Copperfield meets Mr. Micawber, who is later sent to debtors prison. At the factory, he experiences the hardship of the industrial-urban pooruntil he escapes and walks to Dover to meet his aunt. She adopts him and brings him up (renaming him Trot). After finishing his schooling, he goes to London to seek a career and meets James Steerforth and introduces him to his adoptive family. At around this time, he also falls in love with a young girl, the daughter of a well-renowned solicitor. He also meets Tommy Traddles who is boarding with the Micawbers, bringing the delightful but economically useless character back into the story. In time, Doras father dies and she and David can be married. However, money is very short and David takes up various other jobs in order to make ends meet includinglike Dickens himselffiction writing. Things are not well with a friend from home – Mr. Wickfield. His business has been taken over by his evil clerk, Uriah Heep, who now has Micawber working for him as well. However, Micawber (along with his friend Tommy Traddles) determines to expose the bad dealings with which Heep has been taking part and finally, has him thrown out,  returning the business to its rightful owner. However, this triumph cannot be truly savored because Dora has become incredibly ill after losing a child. After a long illness, she finally dies, and David travels to Switzerland for a number of months. While hes traveling, he realizes that he is in love with his old friend, AgnesMr. Wickfields daughter. David returns home to marry her. A Coming of Age Story David Copperfield is a long, sprawling novel. In keeping with its autobiographical genesis, the book has a certain feel for the ungainliness and largeness of everyday life. In the earlier parts of David Copperfield, the novel has all of the power and resonance of Dickens social critique of a Victorian society that had very few safeguards against the mistreatment of the poor and, particularly in its industrial heartlands. In the later parts, we get Dickens most realistic and touching portrait of a young man growing up, coming to terms with the world and finding his literary gift. Although it certainly portrays Dickens comic touch to the full, it also has a  seriousness that is not always apparent in some of Dickens other books. The difficulty of being an adult, of marrying, of finding love and of getting on feel very real and shine from every page of this delightful book. Full of lively wit and Dickens characteristically finely tuned prose, David Copperfield is an excellent example of the Victorian novel at its height and Dickens master of it. Popular (like so many of Dickens works), it has deserved its sustained reputation through the twentieth and into the twenty-first century.

Because this joint carries much of the body weight, and beside the fact that it was designed to do much of the human activities, the knee joint accounts for the most common joint injuries. Its two primary motions are flexion and extension, and a little rotational and lateral motion. â€Å"It is also because of this design that the knee is so vulnerable to injury.† (M. Mullin and K. Stone)   The knee has basically five major parts: the bones, the muscles, ligaments, cartilage and tendons. It is the function of the bones to support the knee and give the joint a rigid structure. There are four bones that make up the knee: first, the femur, which is also referred to as the thighbone or the upper leg bone. (University of Utah, Heath Sciences Center, 2003) This large bone is attached by the ligaments and a capsule to another bone the tibia. Tibia is the larger bone of the knee which is called the shin bone. Parallel to the tibia is another bone called the fibula located below the tibia. The knee cap or the patella is another bone that consists the knee which â€Å"rides on the joint as the knee bends.† (Center for Orthopaedics and Sports Medicine, 1999)   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   It is the function of the muscles to move the joint. The knee joint consists of two kinds of muscles: the quadriceps and the hamstrings. The quadriceps muscles straighten the legs, and are situated at the front of the thighs. The hamstring muscles, which are located at the back of the thighs, are the ones bending the legs at the knee. All groups of muscles are connected to the knee bones by â€Å"thick fibrous cords† called the tendons. (Mayo Clinic, 2006) Patellar tendons connect the quadriceps muscles to the tibia, which are the ones prone to knee joint problems called, â€Å"tendonitis†.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   The bones of the knee are connected by ligaments, which functions are basically to stabilize and hold the joint together. These elastic tissues also provide protection to the joint by limiting the backward and forward movement of the knee. At the center of the joint are the cruciate ligaments called the Anterior Cruciate Ligament (ACL), which is attached to the depression in front of the tibia and â€Å"passes upward, backward and lateral ward and fixed into the medial and back part of the lateral condoyle of the femur.† (H. Gray, 1918) It is considered the key guide wire in the knee joint and is crucial for guiding the tibia in a normal path along the end of the femur. (Mullin and Stone) The Posterior Cruciate ligaments (PCL) protect the joint by preventing the femur from sliding forward on the tibia, and the tibia from sliding backwards n the femur. The media Collateral ligament (MCL) provides protection to the inner part of the knee and the lateral Collateral ligament (CLC) on the outer part. The C-shaped tissue called the meniscus or the menisci cartilage allows the bones to freely slide on each other. They serve as shock absorbers and bone protector. Around the knee joint are little fluid sacs called bursa. These sacs â€Å"help the muscles and the tendons to freely slide as the knee moves.† (Center for Orthopaedics) There are five medial bursas and four lateral bursas.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   The knee is prone to injuries and especially those who are engaged in activities that enable the knee to be overused. These people, most often athletes experience different kinds of pain in the knee joint as a result of injury, stress and wear of the joint. Common to athletes are the ligament injuries called sprain or the injury to the cruciate ligaments. This injury happens when the â€Å"ACL is stretch or torn by sudden twisting motion†, as when the feet are planted one way and the knees are turned another. (Healthlink 2005)   Football players often experience a PCL injury during a football tackle. Contact sports like hockey and football cause injury to the MCL and the CCL, where the said ligament is torn and stretch at the inner side of the knee due to a sudden blow to the outer side of the knee called a contact trauma.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Tendonitis or the jumper’s knee is caused by an overuse of the tendon. As the term implies, this injury is experienced by athletes who are engaged in sports like basketball, which require jumping and running causing a strain in the tendon. Dancers, cyclists, skiers and runners are as victims of this injury. With these activities, the tendon is stretched to the pint that it becomes like a worn-out rubber band and when this happens, it will swell. Muscle contraction happens after a jump or fall, and that repeated force causes strain to the tendons. What is worse is that such contraction, as when trying to break a fall, may tear the quadriceps muscles above and below the patella. Tendonitis causes a person to feel pain during running and jumping or even when in a hurried walking. As a result, the person will feel pain in front of the knee when climbing the stairs or when sitting for a long period. This injury is common to skiers, soccer players and cyclists who, among other athletes are prone to having a blow in the knee cap resulting to a tear of the cartilage. Such contacts can also tear off a large fragment of the cartilage containing a piece of bone.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Injury to the meniscus typically results from â€Å"a twist or torque to the knee while the foot stays still.† (Mullin and Stone) A tear in the meniscus may cause the joint to lock especially when there is a flap of the torn cartilage. When there is only a small tear, mild pain and moderate swelling of the knee lasts up to 48 hours; however large lengthwise tear causes the joint to lock making hard for the person to straighten the knee.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Distance runners are prone to Illiotibial Band Syndrome which happens when the ligament in the illiotibial band becomes tight enabling it to rub against the outer portion of the femur. Pains caused by this injury are felt when waking and when going up and down the stairs. Also associated with running activities is the Osgood-Schatter Disease, where an inflammation of the patellar tendon is involved.   This injury happens when the tendon is stretch too much causing it to tear away from the tibia and in some cases takes a bone fragment. Eugenics is the study or belief in the possibility of improving the qualities of the human species. Sounds good, right? But the question here is, is it moral to sacrifice someones life or the ability for someone to create life in the name of science? Surely Francis Galton and Gregor Mendel thought so. In the nineteenth century, biology was at its peak. Charles Darwin, who just happens to be Galton’s cousin, had just introduced his idea of survival of the fittest. Galton then took that thesis and dissected it. The result of that mess was what we know today as eugenics. The initial proposal was for genetically perfect people to reproduce with others of the same kind. It was believed that mixing the inferior people and the so called perfect beings would â€Å"yield a corruption of blood†(Lombardo,8). It was not until the Civil War ended when America started exploring this new world of genetics. People came up with the belief that â€Å"genetics appeared to be the cause behind human social problems - such as pauperism, nomadism, criminality,alcoholism, feeble-mindedness, rebelliousness and prostitution - as the inheritance of defective germ plasm† (Allen,4). As bizarre as this sounds, eugenicists and their wealthy followers swore by this. With this idea in mind, doctors in prison and mental institutions started sterilizing the inmates and patients. Their reasoning for doing this was that it was better to sterilize these genetically impaired people and let them go knowing there won’t be any other generations of that same problem than to keep them locked up under taxpayers dollar. How could this have been moral? Even Galton, the man who officially named this branch of science called this negative eugenics. We Have Become Untrue to Ourselves

With all the force and vigor at my command, I contend that we have relaxed our vigilance, that we have allowed ourselves to deteriorate. I contend that we have lost our pride in the Philippines, that we no longer consider it a privilege and an honor to be born a Filipino. To the Filipino youth, nothing Filipino is good enough any more. Even their Filipino names no longer suit them. A boy named Juan does not care to be called Juanito anymore. No, he must be Johnny. A girl named Virginia would get sore if she was nicknamed Viring or Binang. No, she must be Virgie or Ginny. Roberto has become Bobbie; Maria, Mary or Marie. And because they have become so Americanized, because they look down on everything Filipino, they now regard with contempt all the things that our fathers and our fathers’ fathers held dear. They frown on kissing the hands of their elders, saying that it is unsanitary. They don’t care for the Angelus, saying that it is old-fashioned. They belittle the kundiman, because it is so drippingly sentinmental. They are what they are today because their elders – their parents and their teachers – have allowed them to be such. They are incongruities because they cannot be anything else! And they cannot be anything else because their elders did not know enough, or did not care enough to fashion them and to mold them into the Filipino pattern. This easing of the barriers that would have protected our Filipinism, this has resulted in something more serious, I refer to the de-Filipinization of our economic life. Let us face it. Economically speaking, we Filipinos have become strangers in our own country. And so, today, we are witnesses to the spectacle of a Philippines inhabited by Filipinos who do not act and talk like Filipinos. We are witnesses to the pathetic sight of a Philippines controlled and dominated and run by non-Filipinos. We have become untrue to ourselves, we have become traitors to the brave Filipinos who fought and died so that liberty might live in the Philippines. We have betrayed the trust that Rizal reposed on us, we are not true to the faith that energized Bonifacio, the faith that made Gregorio del Pilar cheerfully lay down his life at Tirad Pass.

Definition of Nuclear Fission

In nuclear physics and nuclear chemistry, nuclear fission refers to either a nuclear reaction or a radioactive decay process in which the nucleus of an atom splits into smaller parts (lighter nuclei), often producing free neutrons and photons (in the form of gamma rays), and releasing a very large amount of energy, even by the energetic standards of radioactive decay. The two nuclei produced are most often of comparable but slightly different sizes, typically with a mass ratio of products of about 3 to 2, for common fissile isotopes. 1][2] Most fissions are binary fissions, but occasionally (2 to 4 times per 1000 events), three positively charged fragments are produced in a ternary fission. The smallest of these ranges in size from a proton to an argon nucleus. Fission as encountered in the modern world is usually a deliberately-produced manmade nuclear reaction induced by a neutron. It is less commonly encountered as a natural form of spontaneous radioactive decay (not requiring a n eutron), occuring especially in very high-mass-number isotopes. The unpredictable composition of the products (which vary in a broad probabilistic and somewhat chaotic manner) distinguishes fission from purely quantum-tunnelling processes such as proton emission, alpha decay and cluster decay, which give the same products every time. Fission of heavy elements is an exothermic reaction which can release large amounts of energy both as electromagnetic radiation and as kinetic energy of the fragments (heating the bulk material where fission takes place). In order for fission to produce energy, the total binding energy of the resulting elements must be greater than that of the starting element. Fission is a form of nuclear transmutation because the resulting fragments are not the same element as the original atom. Nuclear fission produces energy for nuclear power and to drive the explosion of nuclear weapons. Both uses are possible because certain substances called nuclear fuels undergo fission when struck by fission neutrons, and in turn emit neutrons when they break apart. This makes possible a self-sustaining chain reaction that releases energy at a controlled rate in a nuclear reactor or at a very rapid uncontrolled rate in a nuclear weapon. The amount of free energy contained in nuclear fuel is millions of times the amount of free energy contained in a similar mass of chemical fuel such as gasoline, making nuclear fission a very dense source of energy. The products of nuclear fission, however, are on average far more radioactive than the heavy elements which are normally fissioned as fuel, and remain so for significant amounts of time, giving rise to a nuclear waste problem. Concerns over nuclear waste accumulation and over the destructive potential of nuclear weapons may counterbalance the desirable qualities of fission as an energy source, and give rise to ongoing political debate over nuclear power.

In this paper, I intend to discuss the change that happens in the life of Peyton Farquhar from the beginning of the story to the end. That change is one that is hard to discover because of the fact that we know little about him. However, its easy to see that he does go through changes regarding the way he perceives life, love and the world in general. The character is developed, strangely, in the middle of the story. The beginning of the story sets the scene of a man preparing to be hanged for war crimes off of a bridge (928, 1). At first, we get the scene explained to us in great detail but know little about the man being hanged himself. We see soldiers in uniform standing on the bridge. We see the river running underneath the bridge. We are given the details about the preparation for the hanging including the exact way that it is to be done (929, 4). It isn’t until the end of the first section of the story that we begin to understand what is going on in Farquhar’s head. We learn that he is looking around at his surroundings, almost admiring them, until he fixes his last thoughts upon his wife and children (930, 1 & 2). We see that the man standing on the one board keeping Farquhar from his death, ends up stepping aside at the end of the first section (930, 4).
